Heritage Crossing is owned and operated by Valley Regional Home Health and Hospice. Valley Regional Home Health was created in 2013 by Mari Zakaryan, RN, Director Of Nursing and Arman Zakaryan, as the Director of Operations. Not long after, they realized the importance of the continuum of care to better assist patients in need. This led to providing hospice-level of care along with building a state of the art inpatient palliative and hospice care center, which provides services to acute and routine patients giving them the opportunity for the compassionate care they need, in the comfortable home-like environment they want. Thus, the birth of Heritage Crossing Palliative and Hospice Care Center. Heritage Crossing, the Valley’s premier and unparalleled palliative and hospice care center, will open in the fall of 2019.

This refreshing approach to hospice care center with capacity of 24 private rooms will be cared by a  team of physicians, nurses, chaplains, aides, social workers, and volunteers. Heritage Crossing will be a place that patients and their families and friends can be and where they get to be together for those precious days of their lives while being compassionately looked after.

For Palliative and Hospice Care Services Heritage Crossing is excited to partner with UCSF – Palliative Division Medicine team, to finance a fellowship program as of fall of 2019. UCSF Hospice and Palliative Medicine’s Fellowship program trains top physicians in a fully trans-disciplinary environment. The program’s aim is to help physicians become more compassionate and clinically excellent leaders in all areas of Hospice and Palliative Medicine. This will allow Heritage Crossing to have access to the UCSF Palliative Medical Team, consisting of Palliative Certified Doctors, Nurse Practitioners along with the full collaboration of all Medical Directors of Valley Regional Hospice. This program will be fully supervised by Dr. Patrick Macmillan, (Chief of Hospice and Palliative Care, UCSF Fresno, Associate Clinical Professor, UCSF).
